Nomination Form
The 2008
AMDIS Awards for excellence, outstanding achievement and special recognition
in
Applied Medical Informatics presented by the Association of Medical
Directors of Information Systems.
These
awards honor individuals and/or organizations for successful application
of information systems/technology in the practice of medicine.
Judging
Criteria:
Each
entry will be judged individually by a panel of nationally prominent
physicians and healthcare leaders.
The
criteria include:
- Demonstration
of a successful medical information systems/technology application

Use of information systems/technology to improve patient care
- Use
of information systems/technology to support a more effective
medical practice
- Use
of information systems/technology to improve work flow, enable
efficiencies and/or contain healthcare costs
Winners
will receive:

National recognition in Modern Physician magazine
-
Recognition by AMDIS at the 17th Annual Physician-Computer Connection
Symposium, and on the AMDIS website

A commemorative plaque
